Nopak 4-way cylinder control valve. Inlet, outlet and two cylinder ports are all 1/2" NPT. Configuration and dimensions are given in the pictures. Bottom casting is marked "1137", top casting "1138".
This valve appears to be in good working condition, but it was not tested under pressure.
